If yours is lost or damaged, getting a replacement depends on where you got your shot.If you visited a state-run fixed site, you can call 211 for a replacement card, which will be sent out within one to two weeks. A request can also be made to the Department of Health and Human Services via email at covidvaccinescheduling@dhhs.nh.gov. You can also visit vaccines.nh.gov to download and print a copy of your vaccination record immediately. If you received your vaccine at a pharmacy, you also have multiple options. CVS Pharmacy said patients can access vaccine records via the CVS app or online at cvs.com. Any CVS team member can also print a vaccine record. The pharmacy said a copy of vaccine proof is also emailed when a patient is vaccinated. Walgreens offers a similar service. The company said it can provide electronic copies of COVID-19 dose cards, and it can verify vaccination status on behalf of patients in the event that a dose card is lost or damaged.Manchester health officials said anyone who was vaccinated at one of the city’s clinics can get a replacement card at the Manchester Department of Health.
